Is Psychotherapy Covered by OHIP? An Ontario Insurance and Benefits Guide

When considering therapy, financial questions are often the first hurdle. Navigating the difference between public health coverage and private insurance in Ontario can be confusing, especially when terms like psychologist, psychotherapist, and psychiatrist are used interchangeably.
Understanding how coverage works helps you plan your care without unexpected expenses.
Does OHIP Pay for Psychotherapy?
The short answer is: rarely for private practice therapy. Under the Ontario Health Insurance Plan (OHIP), mental health services are directly covered if they are provided by a medical doctor, such as a family physician or a psychiatrist. However, private psychotherapy provided by a Registered Psychotherapist (RP) or a Registered Social Worker (RSW) in an independent clinic is not funded by OHIP.
While some hospitals and community family health teams offer OHIP-funded counselling programs, these public options often carry months-long waitlists and typically limit care to short-term, solution-focused blocks of four to six sessions.
Extended Health Benefits and Private Insurance
The vast majority of clients in private psychotherapy fund their sessions through extended workplace benefits or private insurance plans. Most major Canadian insurers provide annual mental health allowances that cover Registered Psychotherapists licensed with the College of Registered Psychotherapists of Ontario (CRPO). Common insurance providers include:
- Sun Life
- Manulife
- Canada Life
- Green Shield Canada
- Blue Cross
- Desjardins Insurance
Mental health coverage is typically listed under paramedical services. Check your benefits booklet or member portal to verify your annual maximum, whether your coverage resets on January 1 or a rolling calendar year, and whether your policy specifically covers Registered Psychotherapists.
Direct Billing Versus Reimbursement
In most private therapy clinics, payment is completed at the end of each appointment. You are issued an official receipt containing your therapist's name, regulatory registration number (CRPO), session date, and fee paid. You can upload this digital receipt directly to your insurer's mobile app or portal. Reimbursement is typically deposited into your bank account within two to three business days.
Medical Expense Tax Deductions
If you exhaust your workplace benefits or pay out-of-pocket, keep your receipts. In Ontario, services provided by Registered Psychotherapists qualify as eligible medical expenses under the Canada Revenue Agency (CRA) Medical Expense Tax Credit (METC). You can claim these fees on your annual tax return to offset taxable income.
